About the role

The Dementia Consultant ensures the delivery of the Dementia Support Australia (DSA) service, that includes assessment, case management, supporting, mentoring and modelling to reduce the impact of behaviours on people living with dementia, their carers and staff.

This particular role will be working on the ARCTS program of works – the Acute to Residential Care Transition Scheme. It is a pilot program that we hope will grow and develop into a larger program or works.

Key Accountabilities include

  • Providing dementia specific expert consulting and advisory services within and outside of HammondCare.
  • Developing the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care. This includes the ability to develop and implement client-specific advice and recommendations and address and respond to changes in behaviour.
  • Assessing clients with behaviours related to dementia and providing resources and ideas for effective care provision and support for that persons needs.
  • Accessing current evidence-based guidance and knowledge and ensuring it is aligned with the Dementia Centre and utilising to inform all activities in the role.

To succeed in this role, you’ll need

  • Relevant qualifications in Nursing, Allied or Mental Health
  • Expertise in supporting people living with dementia
  • Experience in Australian residential aged care and or community aged care
  • High level of communication and relationship building skills
  • Problem solving and decision-making skills relevant to the aged and dementia care context
  • A current Australian drivers licence and willingness to travel
  • Empathy and patience

The main requirement for the role is prior experience creating person centric care plans for people living with Dementia. We would consider people from an Allied Health, Nursing, Aged Care or Mental Health backgrounds.
