Post Specific Related Information: The purpose of the CNM2-Enhanced Care Coordinator is to manage and lead a high quality service for vulnerable Patients requiring enhanced care. The Enhanced Care Model was created using the evidence and learning arising from the implementation of the Framework for Safe Nurse Staffing and Skill Mix in General and Specialist Medical and Surgical Care Settings. Enhanced Care refers specifically to the need for additional or extraordinary care, beyond what is provided for within average daily staffing level. The post holder will support and manage the Enhanced Care Service within Our Lady of Lourdes Hospital and engage in future developments in Louth County Hospital & the Cottage Hospital.
